Il Fatto Quotidiano: Sempre meno oneri e maggiori onori: il medico di base vuole demandare tutto?

Da tempo dico che la medicina di base deve essere rifondata con una facoltà apposita che in cinque anni possa “sfornare” veri medici di medicina generale pubblici allocati in tutte le strutture ospedaliere.
Ma loro resistono. Da un lato il ministero della Salute permette il lavoro fino a 72 anni con il decreto milleproroghe, dall’altro li vorrebbero obbligare ad esercitare parte del loro lavoro nelle Case di Comunità, isole nel deserto inutili. Ma la classe specifica è molto forte e, spesso appoggiata dagli Ordini dei Medici di cui eleggono il Presidente ed i consiglieri, si mettono, appena possono, di traverso per lavorare sempre meno.
In questa ottica è da considerarsi la delibera dell’Ordine dei Medici di Milano in cui anche i liberi professionisti sono tenuti a provvedere di persona al caricamento dell’eventuale certificato di malattia sul portale Inps attraverso il sistema della tessera sanitaria. Sempre meno oneri e maggiori onori richiesti nelle poche ore che dedicano alla spesa sanitaria enorme che chiedono alle Regioni ed a tutti noi.

Ma non desidero criticare oltre chi si vuole scrollare di dosso sempre maggiori responsabilità mediche e di controllo della salute lavorativa dei cittadini. Un paziente ha la tosse? Prescrivono la visita pneumologica e si liberano di ogni rischio diagnostico e legale.
Proprio perché non desidero criticare, ma discutere e cercare di risolvere, basterebbe avere un sistema di dati sanitari in qualunque parte del mondo dove ognuno scrive ciò di sua pertinenza ed il paziente ha il compito, di suo esclusivo interesse, di permetterne la visione da remoto dei referti che gli servono anche da un punto di vista lavorativo. Noi tutti dovremmo solo fare i medici, fare una diagnosi ed una terapia e vi assicuro essere molto difficile, anche dopo 45 anni di lavoro.
Enzo Jannacci, noto medico di base, diceva: “Eh, eh, eh, ma se me lo dicevi prima; Eh, se me lo dicevi prima; Come prima; Ma sì se me lo dicevi prima; Ma prima quando; Ma prima no”. Ecco io ve lo dico da tempo e potete ascoltarlo anche nell’ultimo video del mio canale TikTok.

Less and less burdens and more and more honors: does the family doctor want to delegate everything?

For a long time, I have been saying that primary care must be rebuilt with a dedicated faculty that, in five years, could “produce” real public general practitioners allocated to all hospital facilities.
But they resist. On one hand, the Ministry of Health allows work up to 72 years with the Milleproroghe decree, and on the other hand, they want to force them to practice part of their work in Community Houses, useless desert islands. But the specific class is very strong and, often supported by the Medical Orders, of which they elect the President and councilors, they get in the way as much as they can to always work less.
In this context, it should be considered the resolution of the Order of Doctors in Milan, in which even self-employed professionals are required to personally upload any sick leave certificate to the Inps portal through the smart card system. Less and less burdens and more honors requested in the few hours they dedicate to the enormous healthcare spending that they ask of the Regions and of us all.

But I don’t want to criticize further those who want to constantly evade greater medical and legal responsibilities and control of citizens’ work health. Does a patient have a cough? They prescribe a visit with a pulmonologist and get rid of any diagnostic and legal risk.
Precisely because I don’t want to criticize, but to discuss and try to solve, it would be enough to have a health data system in any part of the world where everyone writes what is relevant to them, and the patient has, exclusively in his interest, the task of allowing its remote viewing of the reports that he needs, also from a work perspective. We should all just be doctors, make a diagnosis and a therapy, and I assure you, it’s very difficult, even after 45 years of work.

Il Tempo: Referendum, nasce il comitato dei «Sanitari per il Sì»

Si ritrova il comitato dei «Sanitari per il Sì». Nel pomeriggio di ieri, nella capitale, si tiene il primo incontro dell'associazione che riunisce medici, infermieri, amministrativi, impiegati e operatori del terzo settore favorevoli alla riforma della giustizia voluta dal ministro Nordio. «L'obiettivo dell'incontro - spiega Annamaria Colao, tra le promotrici dell'iniziativa - è mettere insieme tutti quei professionisti che ritengono importante e auspicabile un cambiamento in materia di giustizia».
Diversi i rappresentanti di categoria presenti. Tra questi spiccano: Antonio Magi (presidente dell'Ordine dei medici e degli Odontoiatri di Roma), Carlo Ghirlanda e Marco Landi (Associazione Dentisti Italiani), Piero Ferrante (Federazione Nazionale Ordini Fisioterapisti), Stefano Barone (diregente nazionale del Nursind) e Roy De Vita, noto primario di Chirurgia plastica e riproduttiva dell'Istituto nazionale dei tumori Regina Elena, collegatosi da remoto.
Per quanto concerne la politica, invece, prendono la parola: Sara Kelany, responsabile del dipartimento immigrazione e deputata di FdI e Francesco Zaffini, presidente della Commissione Affari sociali, sanità e lavoro del Senato. Le conclusioni sono affidate al ministro della Salute Oreste Schillaci che, però, chiarisce come abbia preso parte alla riunione a titolo personale e non come rappresentante del governo. Ospite esterno del confronto, invece, il costituzionalista Nicolò Zanon, presidente del comitato Sì Separa, che, in qualità di esperto, spiega le ragioni per cui occorra votare in modo favorevole ai quesiti del 22 e del 23 marzo. «Questa presa di posizione - spiega Kelany, parlamentare di Fratelli d'Italia - è importante perché testimonia come da parte di tutta la società civile ci sia una diffusa volontà a superare lo status quo per una giustizia più giusta. Sempre più persone comprendono come quanto proposto dall'esecutivo sia l'unica strada per migliorare un qualcosa che così com'è non funziona abbastanza. Non mi sorprende, quindi, che la separazione delle carriere, il sorteggio dei membri togati del Csm e l'Alta Corte Disciplinare, pur essendo tematiche che all'apparenza poco hanno a che vedere con la salute, interessino tanti professionisti responsabili e sensibili verso un qualcosa che davvero può  migliorare la vita di ogni singolo cittadino».

Referendum, the committee of “Health Professionals for Yes” is formed.

The “Healthcare for Yes” committee was revived. Yesterday afternoon, in the capital, the first meeting of the association, which brings together doctors, nurses, administrators, employees, and third sector operators favorable to the judicial reform proposed by Minister Nordio was held. “The objective of the meeting – explains Annamaria Colao, one of the initiators – is to bring together all those professionals who believe it is important and desirable to change things in the area of justice.”

Several representatives of professional organizations were present. Among them stand out: Antonio Magi (president of the Order of Doctors and Dentists of Rome), Carlo Ghirlanda and Marco Landi (Italian Dental Association), Piero Ferrante (National Federation of Physiotherapy Orders), Stefano Barone (national director of Nursind) and Roy De Vita, a well-known plastic and reproductive surgery head at the Regina Elena National Cancer Institute, connected remotely.

As for politics, the following spoke: Sara Kelany, head of the immigration department and MP for FdI, and Francesco Zaffini, president of the Senate’s Commission for Social Affairs, Health and Labour. The conclusions were entrusted to Minister of Health Oreste Schillaci, who clarified that he had attended the meeting in a personal capacity and not as a representative of the government. Nicolò Zanon, constitutionalist and president of the Yes Separate committee, was also a guest of the discussion, explaining the reasons why it is necessary to vote favorably on the questions of March 22nd and 23rd. “This position – explains Kelany, a parliamentarian for Brothers of Italy – is important because it testifies to the widespread will of civil society to overcome the status quo for a more just justice system. More and more people understand how much proposed by the executive is the only way to improve something that doesn't function sufficiently as it is. It does not surprise me, therefore, that the separation of careers, the drawing of lots of judges from the CSM, and the High Disciplinary Court, although seemingly having little to do with health, interest many responsible and sensitive professionals who can truly improve the lives of every individual citizen.”
