Impressive blood pressure reduction in Phase 2 trial of baxdrostat, an inhibitor of aldosterone synthesis. Study population had resistant hypertension (we see a lot of these patients in hospital practice). Not associated with gynecomastia (unlike aldosterone antagonists), and had minimal effects on cortisol synthesis. Hypotension and hyperkalemia occurred in some patients. https://www.nejm.org/doi/full/10.1056/NEJMoa2213169

Starting the morning in the hospital today, doing some post it note reverse teaching. Each student needs to write one clinical pearl that he (both dudes today) learned from the morning whilst seeing or reading about his patient. Needs to fit on a post it. Let’s see if I can learn something new. Or at least encourage some learning in bite sized pieces.

Editorial in Lancet urging that all chronic kidney disease patients should get flozins, based on a new meta-analysis showing that SGLT2 inhibitors reduce CKD progression by 37%. Benefit seen in patients with *and* without diabetes. Link to meta-analysis: https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(22)02074-8/fulltext

Is #Twitter doomed?

Here's a Victorian alarm clock which served as a “memento mori,” reminder of death, in the 19th century. It prompted people to think about the shortness of life. This incredible object is now housed at the Science Museum in London.

The data on pandemic control measures (lockdown and masking) being effective against all respiratory diseases is pretty convincing.

An #introduction for the #twittermigration crowd. I'm a hospitalist leader who is Dean for Medical Education at University of Chicago Pritzker School of Medicine. I was an early tweeter on #MedEd hashtag and started #jhmchat for Journal of Hospital Medicine. I’m also an NIH funded researcher who works on improving the learning environment and patient experience, improving diversity of our workforce, and teaching/supporting clinicians to address misinformation. Looking forward to engaging here.

⚠️The bacteria most likely to have induced AmpC β-lactamase are Enterobacter cloacae, Klebsiella (Enterobacter) aerogenes, and Citrobacter freundii. DO NOT use ceftriaxone and ceftazidime against these bacteria, even if they test susceptible. Cefepime is OK if MIC ≤2 mcg/mL. If higher, use a carbapenem

Antibiotic resistance due to AmpC β-lactamases may not be apparent when initial resistance testing is done. It may only be provoked by exposure to antibiotics 💊 (induced chromosomal resistance)

Which of the following bacteria is most likely to produce an AmpC beta-lactamase?

Citrobacter youngae

Enterobacter cloacae
