Impressive blood pressure reduction in Phase 2 trial of baxdrostat, an inhibitor of aldosterone synthesis. Study population had resistant hypertension (we see a lot of these patients in hospital practice). Not associated with gynecomastia (unlike aldosterone antagonists), and had minimal effects on cortisol synthesis. Editorial in Lancet urging that all chronic kidney disease patients should get flozins, based on a new meta-analysis showing that SGLT2 inhibitors reduce CKD progression by 37%. ⚠️The bacteria most likely to have induced AmpC β-lactamase are Enterobacter cloacae, Klebsiella (Enterobacter) aerogenes, and Citrobacter freundii. DO NOT use ceftriaxone and ceftazidime against these bacteria, even if they test susceptible. Cefepime is OK if MIC ≤2 mcg/mL. If higher, use a carbapenem

Antibiotic resistance due to AmpC β-lactamases may not be apparent when initial resistance testing is done. It may only be provoked by exposure to antibiotics 💊 (induced chromosomal resistance)

I thought the squirrels and raccoons got all my apples this year, but they missed one Black Oxford. This deep reddish-purple apple is an heirloom variety from Maine with sweet/tart flavor, excellent storage tolerance, and good disease resistance.

In Chamberlain's account, which is the primary source for Blood Meridian, Holden has many of the marvelous attributes of his fictional counterpart, but is more weaselly, if equally terrifying. Glanton's gang seems to have been as ghastly and murderous in real life as in McCarthy's novel.

Judge Holden, from Samuel Chamberlain's My Confession: Recollections of a Rogue. "He stood six foot six in his mocassins, had a large fleshy frame, a dull tallow-colored face destitute of hair and all expression." He was beardless, but not bald, as Cormac McCarthy portrayed him in Blood Meridian.
