That "If genitals don't define gender, how does removing them affirm it?" line is like saying (this context would be specific to trans men) "If hair doesn't define gender, how does cutting it short affirm it?" Literally all it is is helping the person feel the most comfortable. Not all trans people actually get surgery but many of them do because it helps them fit in. Kinda like how not all men have long hair and vice versa.

It shouldn't be up to someone else how you live your life.
