As a #civilization, we are getting better and better at doing the wrong thing.

On Defining Systems (1995)
"System: A set of entities that are interdependent with respect to the principle governing the set i.e. the system principle. The system principle is not sui generis to the set but defines a special relation of interdependence between the set and its environment." -- #FredEmery
"If we have a system of improvement that is directed at improving the parts, taken separately, you can be absolutely sure the performance of the whole will not be improved." - Ackoff
Some may say I'm reading to much into this, that maybe they are trying to improve the system.
OK, fine. But if that is the case, why is the effort labeled "DEVELOPER productivity"? As if the developers can be treated as an island? Where is work created, assigned, and approved?
I'd recommend the entire video - it'll explain why focusing on attempts to correct the pieces fail to improve the whole. However, what Ackoff says about automobiles around the 4 minute mark is an illustrative example of what Cilliers is talking about in the quote: https://www.youtube.com/watch?v=OqEeIG8aPPk

Watch the video for much more but the basic idea of #idealized_design is to create a new design for a product, service or the organization based on existing feasibility but without the constraints of the existing setup. Then you can use that ideal to figure out a plan to move from the existing state to that idealized design. Russell #Ackoff co-authored a good book on the topic: Idealized Design...
"explore the method to effectively learn from decisions the organization makes. The idea seem simple but they are powerful.
Preparing a record of every decision of any significance, ones that involve doing something or (of particular importance) ones that involve not doing something.
...
The decision should be monitored to determine whether the expectations are being met..."

This is an excellent description of Systems Thinking.
Definitely worth the 15 minutes, and Dr. Ackoff is funny, too!
This presentation is from a 1994 event hosted by Clare Crawford-Mason and Lloyd Dobyns to capture the Learning and Legacy of Dr. W. Edwards Deming. Russ knew Dr. Deming and speaks here about the difference between "continuous improvement" and "discontinuous improvement" as seen through the lens of systems thinking.
