One of my teachers in college told us endless horror stories from his days doing forensic documentation at a nearby nuclear power plant. In short, nobody knew if or how it worked after decades of undocumented hacking to keep it running.

One of my friends was, at one point in his life, a non-destructive testing professional. Fully certified in all methods and environments.
Including nuclear.
Including Davis-Besse. (He was not involved in the pressure head.)
