@maxleibman Unfortunately this is *horrifyingly* true in the process industries (water & wastewater, petroleum refining, biofuels & chemicals, etc.) . Not only is it responsible for some major accidents (the Texas City refinery explosion comes to mind), it's also responsible for a whole ton of major pains in the ass lol
