Today a friend asked me what was up with #COVID vaccines. Why haven't we been encouraged to get further boosters?

The short answer is that there's a new version coming this fall that targets the now-prevalent XBB.1.5 strain that was not included in previous formulations.

My guess is that this will be made available, but not necessarily recommended, to all US adults. I will be getting it if it is available to me.

This Fall’s COVID-19 Vaccine Will Target Omicron XBB Subvariants—Who Needs to Get It?

This Medical News story discusses the updated COVID-19 vaccines expected in the US this fall.

@ct_bergstrom thanks, been wondering about that very thing lately

@ct_bergstrom And in the meantime, folks 65 and older _are_ encouraged to get an additional booster. https://www.cdc.gov/coronavirus/2019-ncov/vaccines/stay-up-to-date.html

Kind of wonder if we'll end up with a mixed schedule of every year for under-65, twice a year for 65+.

@boredzo @ct_bergstrom The mixed schedule is what Japan has now. I convinced my doctor to get me and my husband booster tickets because we’re aging kindergarten teachers. Glad we did because the country’s in the 9th wave now.

@ct_bergstrom

I wonder when. I'm invited to a big deal family wedding across country and my last shot was the bivalent last fall so I'll be needing something soon

@DeborahForPlus @ct_bergstrom According to the materials presented at VRBPAC last month, both companies (Pfizer and Moderna) were prepared to have something ready for authorization in the fall, but exactly when would depend on the precise decision made with respect recommended strain(s). The great thing about the mRNA platform in general is that it's easy to swap out the antigen, but that's "easy" in biologics manufacturing terms; it still takes time for approval and manufacturing.

@wollman @ct_bergstrom

Yes I understand the advantage... mRNA much closer to manufacturing vs growing old fashioned vaccines

Sounds like I'd have to get what we already have....

@ct_bergstrom for what it's worth, the recommendation in 🇦🇺 was that people 65+ and 18-64 with comorbidities be vaccinated early 2023, and that others can consider a booster based on their own risk profile. https://www.health.gov.au/our-work/covid-19-vaccines/advice-for-providers/clinical-guidance/clinical-recommendations

As I'm a health professional, I've chosen to be boosted again, despite not being in a "recommended" group. Many people I know have done the same. But the overall rates are low: only 3 million out of 20 million 🇦🇺 adults have had a booster in the last 6 months

@ct_bergstrom I imagine there will be new advice soon.

@ct_bergstrom ooof... If the CDC doesn't recommend it, insurance is unlikely to cover it (unless governments get back in the business of buying doses)

Wish we were also focusing on inhaled vaccines in the US 😔

Just anecdotally, the bivalent booster didn't prevent me from getting COVID ~5 weeks later 😬

@supersat @ct_bergstrom

The way it was explained to me, the #COVID vaccine isn't a magic shield that stops the virus from spreading. It just makes it so if we get it, the effects are reduced and you have a better chance of avoiding hospitalization or death.

Every single person I know who got vaxxed and then caught it, lived. Some barely felt it. Others (like me) got our asses kicked by it for weeks, with varying side-effects.

But we all survived. The vaccine did its job.

@raminf @ct_bergstrom oh, yes, I'm well aware. Still glad I had the bivalent booster. I just suspect inhaled vaccines in addition might offer an initial layer of defense.

Mostly I am really hoping my friends who are still quarantining over three years later can start doing social things again without a big risk of them contracting it for the first time.

@ct_bergstrom We haven't been encouraged to get further boosters because pols, stupidly believing that they'll SUFFER for reminding folks to protect their health, are scared to recommend this obvious and essential precaution. Myself, I'm eagerly awaiting another boost.
@ct_bergstrom
I live in France and received all of my available boosters. Was due a booster in early June but none available so just suffering a full strength flu event for the last 10 days while on holiday in Belgium and Prague. Being nearly 70 my system struggles to cope with this vicious virus, make the vaccines available...... We really need them!
@ct_bergstrom here in 🇧🇷 the fifth booster dose is available for the general population from all ages I got mine a month ago (alongside with the one for Influenza).
@ct_bergstrom I know people witch compromised immune systems in the UK are on dose nr 7 by now.
@fj @ct_bergstrom
Only over 80s and those with very compromised immune systems are getting the vaccine in the UK. Not sure what is happening in the Autumn, it may only be over 80s and the compromised ìmmune group who get it then. I am not sure what happens to the rest of us.
@ct_bergstrom
I hope it will be offered to those who want it. I definitely want a booster as soon as it's offered to me.
@ct_bergstrom AU gov are spamming all adults relentlessly to go get a booster if we haven't had one in 6mo+.

@ct_bergstrom
Having just checked in w/ an east coast academic med center w/ good ties to CDC's and FDA's vaccine panels (but about some other issue), I did inquire about COVID vax. I find myself in that "almost high risk" cohort based on metrics, so gathering data/info always seems prudent. Have also not yet been diagnosed w/ COVID (🤞; I still assess large groups and carry and where a mask — N95 or KN95 — when & where I please).

Yours words seem to be the gossip. I'll get it too, when I can.

@ct_bergstrom In the Netherlands people over 60, the vulnerable and healthcare workers will be offered the new vaccin.
I’m in healthcare and surely will take the booster.
#COVID19

@ericschutte @ct_bergstrom

strange times - restricted vaccines 💉access 😳 I don’t understand this.

@tobi82 @ericschutte @ct_bergstrom

Yep.....eugenics of the younger.

@artisanrox @ericschutte @ct_bergstrom

But that’s stupid. I thought the evil 😈 guy’s are smart 🤷‍♂️

@tobi82 @ct_bergstrom Strange times indeed, a few years ago everybody went mad because the advice was to take the vaccin (good times for our #conspiracynuts ) and now they get mad because the gouvernement only wants to vaccinate the risk groups !!!!
Very strange indeed !!! 🤭
Knowing the Netherlands, if you don’t fit in the risk groups but you want to get vaccinated I guess you can get it. So no problem.
@ct_bergstrom when that one comes out, are we finally going to be allowed to choose Novavax? Or are they still going to treat vaccines like printer cartridges, whatever brand you buy first you're stuck with for life?
@ct_bergstrom awesome. i’ll be getting it if it’s available to me. haven’t got covid yet and i’m hoping to keep it that way

@ct_bergstrom If it’s not “available” to me I plan to lie and get it anyway.

One of the big takeaways from #covid is that the CDC isn’t going to prioritize our health over, say, economic concerns, so it’s everyone for yourself

@ct_bergstrom Great article, thank you for sharing it.
@ct_bergstrom
I'm lucky to be old enough that I've been granted access to all of the vaccines, thus far. Will be due again in the fall so I'm glad that they're upgrading.
@ct_bergstrom I'd been assuming we're now in "annual booster" territory and I'm anticipating getting one coincidental with a flu shot in the fall. Does that seem about right?

@ct_bergstrom

The vaccines and, for perhaps 3/4 of the US population, prior infection, have held up remarkably well across variants.

I think this, along with increasing government neglect of public health, have contributed to the lack of booster campaigns.

"Across age groups, VE was generally similar against BA.5-related infections and XBB/XBB.1.5-related infections."

Early Estimates of Bivalent mRNA Booster Dose Vaccine ...

This report describes how a bivalent mRNA booster dose provided additional protection against symptomatic XBB/XBB.1.5 infection for at least the first 3 months after vaccination in persons who had previously received 2–4 monovalent vaccine doses.

Centers for Disease Control and Prevention
@ct_bergstrom
At some point it was claimed the vaccine becomes less and less effective after the first boost.
What happened with that?
@torbalan @ct_bergstrom It was the mutation of the virus that caused less preventative effect from the vaccines. The next vaccine has a newer strain added to the Omicron of the last batch.

@ct_bergstrom I'm planning to get it as well. I inherently distrust the general populace these days...
