What frustrated me last time w/ Optus: there never was any introspection on why very capable network designers and managers collectively allowed their platform to ignore best practices.

Designers and engineers make mistakes all the time, but this was a failure of culture. No one spoke up about something that should have been obvious to any senior network professional.

Here we are 2yrs later and ppl have died bc the 000 call path wasnt resilient enough. The senior and mid lvl culture for their network division is clearly rotten.
