<p xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" class="first" dir="auto" id="d1335651e108"/><p xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" dir="auto" id="d1335651e109">Infective endocarditis (IE) is a life-threatening condition that may lead to systemic complications, including diffuse alveolar hemorrhage (DAH), a rare but serious pulmonary manifestation. We present a case of a 37-year-old male with a history of heavy smoking, alcohol consumption, and prolonged occupational exposure to cotton dust, who developed prolonged fever, cough, hemoptysis, and dyspnea. Clinical examination revealed clubbing, pedal edema, raised JVP, bilateral lung crepitations, and a holosystolic murmur. Blood cultures confirmed Streptococcus viridans, and echocardiography demonstrated vegetations consistent with IE. Bronchoalveolar lavage showed sequentially hemorrhagic return with hemosiderin-laden macrophages, confirming DAH. The patient was managed with intravenous antibiotics, supportive therapy, and close hemodynamic monitoring. This case highlights the importance of early recognition and multidisciplinary management of DAH complicating IE to improve patient outcomes. </p><p xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" dir="auto" id="d1335651e111"/><p xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" dir="auto" id="d1335651e112"/>
