(And yes, I recognize how privileged I am that I can rail about the lack of education and experience in #stationaryDesign and #Usability, while others near me fight for survival and against deportation, and a bad international trade deal is going to cause huge unemployment, which could, potentially, force me and my family to leave the country.)
I'm less upset about the office worker creating an unusable form. I'm more upset about the schools and associated organizations doing it. Those people ought to teach our kids. And what example do they provide? A useless form?

Other example: my kid's school organized a field day. During that, some kind of popsicle would be handed out. School asked for permission to have the kid receive and eat popsicle. To supply our permission, we get to fill out a permission slip. It's a form. Which is attached to a letter. It has a cutting hint that we use to separate letter from form. Now the form no longer indicates its purpose. What permission does it extend? No clue. #stationaryDesign #usability
Other example: my kid's school held a fund raiser (because public schools here in the greatest country in the world don't receive enough funding to properly pay their teachers and school supplies). We're buying fruit plants for our balcony. We get to announce our purchase order using a form. It is attached to a larger letter. There's a cutting hint that we use to separate letter from form.
