A 1975 concept drawing for the original Star Wars by Ralph McQuarrie (image 1), showing Princess Leia being menaced by a torture droid in an Imperial prison cell.

Notice in the closeup (image 2) that Leia's left breast has fallen out of her shirt.

JW Rinzler used this image as the frontispiece of his 2010 book The Making of The Empire Strikes Back. I imagine he was quite amused to have Leia's wardrobe malfunction hiding in plain sight.
