Promoted a replica, first insert: duplicate key violation. The #PostgreSQL sequence gap - it's been there since PG10 in 2017.
Sequences are non-transactional; logical decoding is transactional. They don't fit the same model. Tomas Vondra had a patch committed in PG16. It was reverted before release.
Postgres 19 ships the fix. Run ALTER SUBSCRIPTION migration_sub REFRESH SEQUENCES; before cutover.
