Roma, 4 giu. (Adnkronos salute) - “In Europa ci sono alcuni Paesi un po' più virtuosi nell'utilizzo dei test genomici. Tra questi, ad esempio, la Germania, dove, grazie alle linee guida implementate localmente, c'è un'aderenza massimale alla prescrizione dei test. Penso che dovremmo trovare delle soluzioni operative, nel nostro Paese, che devono essere incorporate nell'ambito della Breast unit, in maniera tale che siano rispettate quelle che sono le indicazioni dell'Istituto superiore di sanità e il Consiglio superiore di sanità: prescrivere sempre, dove c'è indicazione, il test genomico per ottimizzare il trattamento in queste donne e per evitare sovratrattamenti e sottotrattamenti”. Lo ha detto Giuseppe Curigliano, professore ordinario di Oncologia medica al Dipo- dipartimento di Oncologia ed emato-oncologia dell'Università Statale di Milano e presidente eletto Esmo-Società europea di oncologia medica, in occasione dell'Annual Meeting dell'American Society of Clinical Oncology Congresso (Asco) che si è svolto in questi giorni a Chicago (Usa).
“I test genomici rappresentano veramente una grande opportunità per le donne con tumore mammario endocrinoresponsive Her2 negativo in stadio precoce - spiega Curigliano - Purtroppo l'accesso ai test genomici in Italia non è ottimale. Nonostante ci sia un fondo dedicato, non si riesce ancora a saturare completamente, in alcune regioni, il loro utilizzo. Questo purtroppo è un grosso problema, perché rischiamo un sovratrattamento in molte donne in cui non c'è nessuna indicazione alla chemioterapia, e a volte, ahimè, anche un sottotrattamento, perché il test permette di capire bene chi ha indicazione alla chemio e chi no”.

Among these, for example, Germany, where, thanks to the guidelines implemented locally, there is maximum adherence to the prescription of tests. I think we should find operational solutions, in our country, that must be incorporated within the Breast unit, in such a way that they respect those that are the indications of the Superior Institute of Health and the National Council of Health: to always prescribe, where there is indication, the genomic test to optimize treatment in these women and to avoid overtreatment and undertreatment.” This was said by Giuseppe Curigliano, Professor of Medical Oncology at the Dipo - Department of Oncology and Hematology-Oncology of the University of Milan and newly elected President of Esmo - European Society of Medical Oncology, during the Annual Meeting of the American Society of Clinical Oncology Congress (Asco) which took place in recent days in Chicago (USA).
“Genomic tests truly represent a great opportunity for women with hormone-responsive, HER2-negative breast cancer in early stages - explains Curigliano - Unfortunately access to genomic tests in Italy is not optimal. Despite there being a dedicated fund, it has not yet been possible to fully saturate, in some regions, their use. This is unfortunately a major problem, because we risk overtreating many women in whom there is no indication for chemotherapy, and sometimes, sadly, also undertreatment, because the test allows us to understand well who has an indication for chemo and who does not.”

(Adnkronos Salute) - "E' fondamentale sottoporre a biopsia liquida tutte le donne che mostrano una progressione della malattia dopo una prima linea di trattamento con inibitori delle cicline per forme metastatiche. Svolgere la biopsia liquida è cruciale, poiché sappiamo che più del 40% di queste pazienti presenta la mutazione di Esr1: senza questo esame diagnostico la mutazione non verrebbe identificata, negando di fatto l'accesso a una terapia mirata ed efficace". Lo ha detto Giuseppe Curigliano, presidente eletto dell'Esmo - European Society for Medical Oncology e professore del Dipo-Dipartimento di Oncologia ed Emato-oncologia dell'università di Milano, intervenendo all'incontro organizzato oggi a Milano sull'accesso alla biopsia liquida per la medicina di precisione nella cura del tumore al seno metastatico ormonoresponsivo/Her2.
"Medicina di precisione - spiega Curigliano - significa isolare il Dna tumorale tramite biopsia (tissutale o liquida), sequenziarlo e identificare le specifiche mutazioni che rappresentano il bersaglio molecolare per determinate terapie. L'impiego di elacestrant nelle donne con la mutazione è un chiaro esempio di medicina di precisione applicata alla patologia mammaria". Le terapie biologiche "rappresentano una vera rivoluzione nell'ambito del tumore mammario - afferma - perché offrono una straordinaria qualità della vita, una minima tossicità e una compliance che in genere sfiora il 100%. Nello specifico - sottolinea - le terapie orali in questione sono trattamenti definiti tecnicamente biomarker-driven: per poter somministrare un farmaco attivo, è necessario identificare prima una mutazione specifica".
"Tra queste terapie orali - evidenzia lo specialista - va sicuramente menzionato l'elacestrant. Si tratta del primo farmaco della classe degli Oral Serd (degradatori selettivi orali del recettore degli estrogeni) e rappresenta un successo per lo sviluppo e la ricerca italiana, essendo commercializzato da un'azienda del nostro Paese. I pazienti eleggibili per questa terapia sono coloro che presentano una malattia metastatica endocrino-responsiva, Her2-negativa, e che sono portatori della specifica mutazione del gene Esr1, la quale deve essere ricercata tramite biopsia liquida".

Conducting the liquid biopsy is crucial, as we know that more than 40% of these patients present the Esr1 mutation: without this diagnostic test, the mutation would not be identified, effectively denying access to a targeted and effective therapy.” This was stated by Giuseppe Curigliano, newly elected president of the ESMO - European Society for Medical Oncology and professor of the Dipo-Department of Oncology and Hematology of the University of Milan, speaking at the meeting organized today in Milan on access to liquid biopsy for precision medicine in the treatment of hormone-responsive/Her2-negative metastatic breast cancer.
“Precision medicine – explains Curigliano – means isolating tumor DNA via biopsy (tissue or liquid), sequencing it, and identifying the specific mutations that represent the molecular target for certain therapies. The use of elacestrant in women with the mutation is a clear example of precision medicine applied to breast pathology.” The biological therapies “represent a true revolution in the field of breast cancer – he states – because they offer extraordinary quality of life, minimal toxicity, and compliance that generally approaches 100%. Specifically – he emphasizes – these oral therapies are technically biomarker-driven treatments: to administer an active drug, it is necessary to first identify a specific mutation.”
“Among these oral therapies – the specialist highlights – elacestrant must certainly be mentioned. It is the first drug in the Oral Serd class (selective oral estrogen receptor degraders) and represents a success for the development and research in Italy, being commercialized by a company in our country. Patients eligible for this therapy are those who present with an endocrine-responsive, Her2-negative metastatic disease, and who carry the specific Esr1 gene mutation, which must be identified through liquid biopsy.”
