Disasters will happen. Only one position is really OK (see alt text/caption) Only some millimeters up or down = completely closed, will do.
Plus: Some chucks are fine, others sucks.

@[email protected] Make the grasp, or whatever you call it, of the jaws on your chuck thighter. Perfect circle. Learned that from Glenn Lucas. Or a bigger chuck with thighter grip. The big opening you had damaged the tenon, biting in on the corners of the jaws, and may have been part of the disaster.
