Dialysis delay at what cost? Protein restriction as a distraction in the age of GDMT β€” NephJC

The NephJC shorts on cost-efficiency of low-protein diets and ketoanalogues as a distraction from CKD-relevant therapies in the 21st century… wait, only if the comparator wouldn’t be dialysis.

To me, it is difficult to justify a placebo controlled trial without proper guardrails (not transfusion - rescue ESA maybe) which was done in the non-dialysis trials In dialysis patients with anemia, how do you justify withholding ESA (which is standard of care)? #NephJC
There was some intriguing data for HIF in inflammation or high hepcidin states - but I haven’t seen good data - I guess the field has moved to trying Ziktekevimab etc instead? #NephJC
Yeah: cost & convenience As @[email protected] said last night in India the HIF is 60% of the ESA The original HIF:ESA trials were designed as non-inferiority for this reason, since the lower cost and pills versus injections would justify an NI design #NephJC
I go up to 1200 based on DRIVE pubmed.ncbi.nlm.nih.gov/17267740/ in @[email protected] #JASN What’s your upper limit of tolerance for ferritin? #NephJC

Ferric gluconate is highly efficacious in anemic hemodialysis patients with high serum ferritin and low transferrin saturation: results of the Dialysis Patients' Response to IV Iron with Elevated Ferritin (DRIVE) Study - PubMed

Few data exist to guide treatment of anemic hemodialysis patients with high ferritin and low transferrin saturation (TSAT). The Dialysis Patients' Response to IV Iron with Elevated Ferritin (DRIVE) trial was designed to evaluate the efficacy of intravenous ferric gluconate in such patients. Inclusio …
