The failure of the primary flight computer on Starship IFT-1 explains a lot. We know that at about 3:12 into the flight the vehicle triggered the flight termination system - but the engines kept firing. Normally the AFTS shuts down the engines at the same time. But without a flight computer the engines were stuck on. That allowed the autogenous pressurization system to keep filling the tanks, keeping them structurally sound for another 40+ seconds into the flight.
SpaceX has an update on the FAA investigation with some more details of what happened on Starship IFT-1. The FAA and SpaceX, along with NASA and the NTSB as observers, have identified 63 corrective actions for the next flight. That does not mean that SpaceX has 63 to do now - most of those corrections have been made already, including extensive pad and vehicle changes.
