CT chest shows a markedly dilated, fluid-filled oesophagus with smooth distal tapering and no obstructing mass—imaging features suggestive of achalasia.

Diagnosis confirmed on high-resolution manometry (Type II).

Patient underwent POEM, with dysphagia score improving
from 3 → 1.

📌 A nice example of radiology–manometry–endoscopy concordance and clinical response.

🦠 Blown-Out Myotomy (BOM) in Achalasia

BOM occurs when a prior myotomy fails, causing recurrent dysphagia, chest pain, and poor esophageal emptying.

A new study shows POEM is a safe, effective salvage option with 85.7% success at 2 yrs, even in altered anatomy—restoring swallowing and quality of life.

@Yell0wbag G’day mate! Fellow #achalasia warrior here. I’ve had POEM and dilations, reached end stage and finally had an oesophagectomy 2 months ago (very big op, 3 months off work, up to 12 months full recovery - best thing I’ve ever done!).
I can’t find much of an Achalasia community here - just us 2 plus another you can find through the hashtags.
But… have you tried FB and Insta? There are some great people on Insta and very well researched groups on FB that can help and…
Has anyone got experience of living with the oesophageal condition #achalasia ? If you do and you have any eating/food tips I would be happy to chat. I've had a balloon dilatation and POEM (14 months ago) but still have problems swallowing food.

#Achalasia of the esophagus is "What is on your mind?" at the moment.

Was real hungry on the way home from the store, so gobbled a little bit from a baguette. Guess I forgot to chew it well enough, so now I wait and wait for it to go down.

It will go down eventually, but it hurts until my body decides to relax enough to let it drop.

Relax, relax, RELAX!!
