[Interest Survey] Researching Indie Gaming Experiences of Queer Players

RESEARCH SUBJECT CONSENT FORM Title: Researching Indie Gaming Experiences of Queer Players Protocol No.: 30426 Investigator: Michel Maksimova Daytime Phone Number: +12679011695 Email: [email protected] RESEARCH CONSENT You are being asked for your consent to take part in this preliminary survey. This consent page describes the key information that we believe most people need to decide whether to take part in this research. Why am I being invited to participate in this research? This survey serves to find players that would like to be interviewed about their gaming experiences for a study. If you identify as a queer/LGBTQ+ video game player, your experience will help us learn more about how LGBTQ+ audiences interact with video games. The results of this study will help connect real players’ experiences with existing theories about video games. How long will I be in this research? The survey will take up to 10-15 minutes to complete. You may be contacted for an interview within 1-2 weeks after completion and will be provided with a separate consent form for the interview. What are the risks of this study? There are no physical risks but you might experience momentary discomfort. You do not have to answer any questions that make you too uncomfortable and may withdraw at any point. There is also a mild risk of breach of confidentiality. Your participation in this survey will be held strictly confidential, however, confidentiality cannot be guaranteed. What happens to the information collected for this research? Your private information may be shared with individuals and organizations that conduct or watch over this research, including, if applicable: - The Primary Investigator: Adrienne Shaw - The Institutional Review Board (IRB) that reviewed this research - Temple University The survey responses will be briefly stored on Google Drive, then transferred to a password-protected folder on a password-protected computer and deleted from Google's servers once the survey closes. The information directly provided in the survey will not be published. We may publish anonymous and highly generalized survey response statistics if relevant to the study.
