@adamshostack from the report: this novel design should have made 100s if not 1000s of test dives with no one onboard.
The Titan was lost with 5 souls onboard on only the 25th dive.

Stockton Rush ignored regulations, best practices, expert recommendations, and fired anyone who didn't agree with him. Incredible hubris.
