Only PhD’s are allowed to use the title of Dr.

Sorry that's not correct, or at least a simplification.

In the UK medical doctors (who actually hold two degrees but not a PhD) are titled Dr.

It's only surgeons whose title is Mr. or Ms. as originally they were 'barber surgeons' so more of a trade than a profession.
