It is so difficult to find a way to make team to be a team, not just a bunch of people working on the same thing.
Here only few of a lot of things I have learnt:
1. Learn a lot (daily)
2. Share a lot. Nothing is to be greedy about here.
3. Trust people and let them work on their own.
4. EVERYONE's opinion ALWAYS matter.
6. Write to team members sumetimes to ask how is it going. Simple "How are you?" can change someone's day drastically.

@bronley Add: Celebrate individual successes. Handle problems, even if doing so is uncomfortable. Accept people's work (don't go redo it yourself) - if it's not good enough, practice encouraging feedback (what's good about it, what can be improved) and ask for it to be fixed. If they still can't do it, they may need some training/classes/help. Empower and inspire. Do some fun team-building activities (field trip, lunch party, birthday parties, secret santa gifts).
@bronley Praise publicly and often. Chastise, suggest better, etc, in private.
@bronley Respect people's time - keep meetings on track and as short as possible.
