Another day, another systemic failure. My supplier offers two types of EV charger; I researched and chose type B. Supplier is v busy and outsourced it. Outsource company repeats the supplier’s survey questionnaires, submits application to DNO. DNO approves the fitting … of type A. So now I have to chase three organisations to correct the fault. #Systemantics Every handoff/outsourcing is an opportunity for miscommunication and drives up failure demand.
