Basically it's the biggest possible clusterfail between division bosses trying to cram features in and lower ranked people having no power or interest in correcting them.
The assumption that someone wanted to make good product is wrong. They just maximized "product" in all possible interpretations.
Happens all the time. ;)
Safety regulations are #writteninblood or they would try to get "creative" in that space as well.
