One last(?) thought. If you break your PDF form with cute GOTO buttons, why not write the form in a Free language, HTML. JavaScript is not necessary.
Adobe Acrobat/InDesign has stayed alive 33 years beyond its relevance by convincing enough people that HTML is just a fad.
