Xcode 26 changed how tabs work (again!) you can fix it by going to Settings -> Shortcuts and A) remove the shortcut for new tab, and B) make cmd T the shortcut for New Window Tab.

Apple: no one wants editor tabs. STOP TRYING TO MAKE EDITOR TABS HAPPEN
