Think what's tripping up people on #matrix and #fediverse is about it being a federated protocol.
So yeah it doesn't talk activitypub but you probably could setup a bridge, doesn't seem to be that intuitive use of it imho.
It's a bit more Slack or Discord like than straight up IRC.

Similar to the "Eternal September" problem Usenet had when AOL made it availible to all of their customers.
But with federated instances there is a chance it can be moderated to not happen.

Yeah i've seen some people overuse Content Warning to some extent, but it is up to the writer if they wanna put it on people,
however it is courteus to do so.
When it comes to new tech it i generally curious people who first use in general Tech interested and artsy ones, after that it tends be out o necessity before it becomes a adopted by bigger popolus.
So imho the topical instance issue is down to mastodon/fediverse isn't that common yet.
