I don't think it's fair to his personality to attribute his lifelong mendacity and criminality to mental illness. Sure, he may have dementia now but that's a late development in a long history of immoral and evil actions.

It may be that dementia is exacerbating his innate immorality and narcissism as the filtering of the frontal lobes fades away and he can no longer limit his hateful urges.
