This is N535PA Clipper Mercury, which was leased from World Airways. This 747-200C (msn: 20651) Convertible model was built for World Airways in 1974 but was then leased to Pan Am Cargo from October, 1974 to December, 1979.

According to the "Mentour Pilot" videos, this cargo nose door was the original plan for all 747s, as they thought passenger planes would be mainly Concorde-type craft. That was apparently why the 747 cockpit is on a higher level, so that it could be easily sold as a cargo plane.
