When your job is to make things better, it's so much easier to build processes and rules to prevent people from doing a bad thing again. Unfortunately, that only holds up as long as someone's willing to and has the power to enforce it. This type of prevention rarely fixes the underlying problem. It'll most certainly surface again, uglier than ever.
