A Naval Court of Inquiry has found that the good ship Manawanui grounded in Samoa because of human error.

The crew didn't realise the ship was on autopilot (doh!) so the vessel ignored their efforts to change direction and sailed happily towards land until it grounded.

It'll be back to Nav-101 for somebody.
