There has been a program to actively monitor for bioterror events in the US since 2003 called #BioWatch. Parts of the program have been unfunded or quietly suspended and it's been moved to cybersecurity (CISA).
With the program functioning, we'd know if a major terrorist attack happened in a matter of hours, without it, we'd find out a few weeks later when thousands (or millions) of people show up in the ER.
