The funniest moment in my whole playthrough probably was when I had equipped Grog, a pet that instakills any party member when they're knocked unconscious. But I didn't read that, of course. I was playing through an arena-based DLC and reloading like crazy because my companions were dying left and right. Luckily, someone had done the same thing and I found their post and realized my mistake.
