The Town of Pigs is very unsettling and gory and creepy. A village is besieged by faceless invaders who transform the villagers into pigs, and a boy tries to escape before he meets the same fate. (I now realize this could've been on the short-form prompt too.)

Paranoia Agent. Satoshi Kon's only anime series. it's psychological horror at its finest. Also the OP is still one of my fave openings ever. So much joy and despair packed into it:
There is no trope that I hate more than people talking about "the thing" or "the plan" or something similar because they don't want to reveal it too early to the audience. Nobody talks like this when all parties talking already know the details. I can't stand how forced the dialogs are.

@To I'd have to say Hell Girl [1] for anime, it made me laugh a lot but it's definitely going for horror.
For manga I'd go with Junji Ito's works [2] , I haven't read all of them but I was going to list several and this is easier. Good starting points are Tomie, Gyo, and Uzumaki
Slightly off topic, but real good is Drill Hole in my Brain
It's part of the Faust anthology [3] of Japanese fiction and it stuck with me in a real way
