- Literally because this was a presser to tout Trump's lowering of GLP-1 drugs, and a GLP-1 patient passed out behind him
- Symbolically because the photo represents a Marie Antoinette-like indifference of Trump to the suffering of Americans in the face of his pro-billionaire austerity administration
