It rarely fails to amaze me how little thought even very large enterprise-level companies put to their form UX. Those radio buttons are wildly confusing, and why can we resize that box? Fun additional fact, when the box IS resized, the content around it does not move out of the way, so you end up typing over the questions below. Big yikes.
