I saw a post last week about form data collection that I can't find...... and I've been tasked to develop a form to capture volunteer data for a group I work with.....

I hope I've got things right, I'm certain that it's orders of magnitude better than the Word document that preceeded it! ;)

The Age 18+ choice triggers some conditional, additional collection later in the process.

CryptPad: end-to-end encrypted collaboration suite

Sort of decided that there is no legal basis to gather address and postcode. It was being captured to supplement capturing DBS info and referees later in the form. Thinking it through, these really need to be split out as not all volunteers will need the DBS check. Some other feedback too, all gratefully received! Thanks.

@gruff In the UK there is no legal difference between "preferred name" and "given name" (or even "legal" name) - if you need a name that appears on some kind of documentation (e.g. because you require cross-referencing with an identity document) then you should state that explicitly - otherwise you can just say "name".

(related: it's generally an anti-pattern to split names along arbitrary boundaries like last/first - there is no requirement in UK law that names be particularly structured)

@sarahjamielewis Thanks so much. I did research and got befuddled. What you say is clear and so obvious really. It's quite remarkable how one becomes 'contitioned' isn't it? Me, I mean! ;)
