@tom_geraghty Every time I read case studies like this I have a visceral hatred of the legal system. These incidents need to go in front of an inquest board to reveal all the systemic organizational problems, the environment, and the history. If malice is found, forward the case to prosecution otherwise leave the criminal justice system out of it. They have one purpose: find an individual to blame. That's anathema to protecting the health and safety if the public.
