I may have griped about this one already: my EHR has a field to enter comments for the pharmacy team. So when I prescribe meds that come as both tablets and capsules, I'll put "may substitute capsule/tablet" in the comment field.

I had yet another prescription rejected by insurance because they'd pay for one but not the other. Called the pharmacy; they don't see that field unless they go thru extra hurdles.

So why is it there?
