Because I'm ignorant:
1) Do all states do provisional ballots?
2) Do all states require you be given one if you are turned away, or are there additional conditions in some places?

BallotPedia explains the process. Idaho, New Hampshire and Minnesota are the only states without Provisional Ballots.

You want to have a receipt as that allows you to be part of the "curing" process after the fact. Elections have been decided by "cured" ballots.
