It's a 1963 silk screen cover by Andy Warhol that was produced on the occasion of the "Popular Image" exhibition at the Washington Gallery of Modrrn Art April-June 1963. In all there were 5 different colors made with 75 copies done of each. According to the catalog raisonné posted above only 75 total had records included with them with rest remaing empty. The rest of the pressing run were packaged in a separate printed Jim Dine designed cover which is in itself quite rare. This is considered his first ever pop art record cover and one of his first pop art pieces in general. The record has no scratches but shows signs of nearly 60 years in a tattered inner sleeve. The cover has overall wear as shown and two seams completely split so it's basically a folder at this point.
