@NicuofThings Since it’s not what I’ve been thaught. What would be the benefit of choosing intubation in this case?
@Gotped There are various protocols and care bundles for managing babies born before 25 weeks. The current trend is to intubate these infants prophylactically at birth for at least 72 hours before considering extubation. For babies born before 24 weeks, some guidelines recommend a longer period of invasive ventilation. What do you do in your current practice?
@NicuofThings
There would likely be a need for intubation after the first honeymoon week, but as for intubation in the delivery room numbers are going down in Sweden at the moment.
@NicuofThings Most are still intubated at some point during their care.
@Gotped The argument for the "intubate prophylactically" team is that when these babies eventually require intubation, they will need higher ventilatory and haemodynamic support compared to being intubated at birth.
@NicuofThings Well it won’t save them from the natural progress of inflammation and RDS-development (yet), it will possibly be a less safe intubation, but there is more time for more gentle recruitment of the lung and you will pass the first few days of IVH risk of the ventilator. Don’t know if that is good or bad to be honest, as with everything in this field.
