@arichtman @tdp_org I don't think this is Jira specific. Person creating the form simply should have used radio buttons instead of checkboxes. I've seen this in plenty forms and questionnaires in multiple platforms. Form creation is often done by non programmers who might not know the difference I suppose. I am sure platforms could make the difference clearer though
