Digital clubbing, look at his fingertips and notice the disparity of color between his red swollen arm and the left pale thin arm. A photo with both arms down would be nice but I don’t it interesting anyhow.
Wouldn’t surprise me if it’s related to malnutrition due to the various famines North Korea has endured. He looks like he probably would have been in his teens or 20’s during the Arduous March famine in the 90’s, so unless he was from a powerful/influential family then he likely suffered from malnutrition while a conscript in the army.
That would certainly make sense.
