@Popehat In healthcare, it's very important to take near-misses very very seriously, because reacting properly to a near-miss is your best chance to fix things without incurring a death toll.

Seems like that should be understood better in law and governance.

@androcat @Popehat Same is true in industrial safety. Near misses need to be analyzed, root causes identified and corrected to prevent a truly damaging event. #Trump for conviction would be a good start. Identifying additional co-conspirators would also be desirable. TY for your posts.
