This is a hex map (1" grid) my father created for a 1st edition #DnD game he ran about 45 years ago.

I have his spiral bound, handwritten notes for the game. There are 108 major locations (edged in different colors), and somewhere between 300 and 500 separate encounters (occurring in individual hexes).

In the burnt ruble of the Temple of Fire rests a small gold tinderbox. Using the tinderbox, such as to start a fire, causes the tiny ember or produces to erupt into a 20d6 fireball. This explains the condition of the Temple of Fire.

Or in the House of Assassins there is a room covered in fresh blood and gore. Clumps of hair and flaps of skin occasionally drop from the ceiling. Propped up against one wall is an impeccably clean teddy bear with a patch sewn on one knee. There is a note to the DM that this is just a recipe for paranoia - the blood is just blood, and the teddy bear just belonged to a previous adventurer with security issues.
