It's a feature built into mastodon. TLDR; Preferences > Account > Moving from a different account / Move to a different account
@charliewilco and I just realized I didn't answer your question but I assume the process moves your posts and followers.
Garrett McCullough@overra @charliewilco your posts do not follow you to the new account but there's a redirect from your old account (that's what happened for me, at least)
Your followers are moved over automatically
The accounts you follow are not moved over but you can export a CSV and import it into your new account
