Environ 25% des receveurs dâorganes prĂ©sentent un rejet aigĂŒ au cours de la premiĂšre annĂ©e post-greffe. Le rejet aigĂŒ peut-ĂȘtre cellulaire (ACR), humoral (AMR) ou mixte. Au long cours, le risque de rejet chronique quelque soit ses formes impacte le fonctionnement et la survie du greffon et du patient. Il est difficile de trouver un Ă©quilibre entre les effets bĂ©nĂ©fiques et nĂ©fastes de lâimmunosuppression pour prĂ©venir ou traiter le rejet. Des facteurs tels que les infections opportunistes et les tumeurs malignes contribuent Ă rendre ce dĂ©fi toujours plus complexe. Par consĂ©quent, les thĂ©rapies qui peuvent moduler les rĂ©ponses allo-immunes sans diminuer la rĂ©ponse immune vis Ă vis des infections et des cancers suscitent un vif intĂ©rĂȘt. La photophĂ©rĂšse (ECP) prĂ©sente un potentiel dans cette approche en transplantation dâorgane mais le mĂ©canisme dâaction prĂ©cis reste Ă determiner. Objectifs dâapprentissage : Fournir une vue dâensemble du rĂŽle de la photophĂ©rĂšse dans la transplantation dâorganes solides et expliquer pourquoi il est important de comprendre son mĂ©canisme dâaction sur le plan clinique. Passer en revue les derniĂšres connaissances sur le mode dâaction de la photophĂ©rĂšse et la façon dont elle est utilisĂ©e pour traiter les receveurs de greffe dans diffĂ©rents scĂ©narios cliniques.
