E.g.
Set up surveillance of a suspected London Tube bomber associate - who'll have to be killed rather than be allowed to get onto a train.
And fail to ensure that there are enough staff on that job to prevent any need to kill the suspect if he goes to catch a train.
But he goes to catch a train. And your staff kill him.
You get charged with administrative manslaughter.
