I've been reconceptualizing MCAS in my practice.
Rather than treating it as a standalone diagnosis, I view it as an "immune derangement syndrome"—the cumulative result of multiple immune insults over time.
Infections. Toxins. Stress. Autoimmunity. They stack.
Treatment approach:

LDN for immune modulation
Ketotifen for mast cell stability
Methylene blue for mitochondrial support
Address root causes

The body doesn't compartmentalize dysfunction. #MCAS #IntegrativeMedicine #FunctionalMedicine

The concept I call "endorphin reserve" has fundamentally changed how I approach LDN therapy.
Before dosing, I assess:

Duration of illness
Sleep quality
Energy levels
Functional capacity

Patients with severely depleted reserves—those who are very ill—need completely different protocols than standard dosing. Sometimes we start at micrograms, not milligrams.
